What is Bioplant® HTR® ?

Bioplant® HTR® - Hard Tissue Replacement®- is a safe,100% synthetic matrix material. It is an alloplast that regenerates autogenous new bone. It is an (almost)* non resorbable, microporous calcium layered co-polymer of polyhydroxyethyl-methacrylate and polymethyl-methacrylate. It is a patented bone regenerating material for dental and orthopedic applications. It is unique among bone substitutes because it promotes the growth of new autogenous bone. It's not only a bone filler, but also a bone promoter. The strong, hollow and safe matrix only takes 10% of the volume that has to be filled f.i. in the jawbone. Through this hollow matrix then, new dense bone grows so bone structure becomes integrally restored. Over the years H.T.R.® resorbs* very, very slowly and 90% of the normal healthy bone structure remains permanently.

What is Advanced Extraction Therapy?

Advanced Extraction Therapy™ is a new way to prevent (the otherwise inevitable) bone loss after tooth extraction, by the use of a bone regenerating material that leaves open all options for future treatment.

When is HTR® a suited choice?

Bioplant HTR® can be used for following treatments:

  • After an extraction, to preserve the height and width of the jawbone (HTR-24®);
  • For immediate implantation (around the implant -HTR-40®);
  • For periodontal treatments (HTR-40®);
  • For augmentation of the resorbed ridge (HTR-24®);
  • for endodontic applications in the mouth and facial surgery(combination of HTR-24® and HTR-40®).

HTR® - 24 or 40: the figure represents the density of the beads per square mm. about 24 or 40 beads/mm˛.

HTR® - 24 is a relative big bead for filling up big voids with a hollow matrix.

HTR® - 40 : is a relative small bead, used when there is little space, f.e. around implants or periodontal areas.

Is HTR® Therapy suited for everybody?

Yes, HTR® Replacement Therapy has a success percentage of 97.9%. It is one of the most used and most safe bone regeneration materials that are enlisted world wide. The treatment is successfully applied since 1975 in more than 100.000 patients, varying from 12 to 88 years of age. This without one single problem or complaint!

Can I apply HTR® in my every day practice?

Yes, for filling up the alveole socket after an extraction. This is quite possible in the chair and is not complicated at all. You don't need any additional equipment. You need at most 5 minutes of extra time. Your patient will be thankful until eternity because this material fits in optimally in the bone tissue and provides for optimal healing.

Why should I use HTR®?

Bioplant HTR® prevents every possible bone loss. In this way you are assured that the jawbone will offer a solid foundation for every sort of treatment later on. This for removable prosthesis as for crown & bridgework as for dental implants. In addition the for the patient so important esthetics from mouth and face is preserved.

Don't I have to use a membrane with H.T.R.®?

The Guided Tissue Regeneration membrane or GTR® (resorbable or non resorbble) is almost never used in combination with Bioplant® HTR®, unless a very large site has to be filled or a treated quadrant has to be especially protected for:

  1. considerable loss of Bioplant® HTR® material due to movements of the tongue of the patient.
  2. infiltration of food into the wound.

Most of the dentists use a material like Gelfoam®, a collagene dressing, a surgical tin-foil or sometimes a big temporary flat pontic. The injurious of the use of a membrane are shown in a study with immediate post-extraction implants (see on our list, study n° 239 of Hahn J.) The simultaneous use of the membrane and HTR in animal studies resulted in a 90% infection ratio. The same results appeared when the membrane was used without a bone filler. The use of Bioplant® HTR®, without a membrane resulted in an infection ratio of less than 1%!

Another reason that pleas against the use of a (resorbable or non-resorbable) membrane is a relative strong acid surrounding that arises in or around the regenerating bone. This slows down the formation of new bone quite considerably, even when the bone substitute is grafted with BMP's (see publication n° 51)

The synthetic alloplast acts, out of his own as a membrane by formation of a dense fibrous tissue at the surface. Eventually, after 3 to 5 years this is transformed in the new regenerated bone. (publications n° 253 en 208).

Can I personally follow up the evolution of healing of HTR® ?

Every bead of Bioplant HTR® contains traces of the radio-opaque barium-sulfate. The evolution of the bone regeneration is very easy to observe in radiographic photographs. At the beginning stage the volume is filled with clearly aligned beads of HTR®.
After a few months the contours of the alveolar walls fade out. This is a sign that shows that new bone has grown into the defect. Eventually almost no HTR® beads remain visible on the radiograph. Only dense secundary bone remains to be seen on those places were HTR® was applied.
After 8 to 15 months only the new dense bone, that is formed in and around the matrix, is visible. The beads are still there, but they are no longer visible. In the course of the following 15 years the become progressively resorbed.

I placed an immediate (right after extraction) implant, in combination with HTR®. How long do I have to wait before it can be fully loaded with a crown?

Dependable of the condition of the bone and the health habits of the patient, a waiting period of 12 to 15 months is recommended due to the time it takes before the beads of HTR are fully grown through with new bone. Also the size of the defect plays a role. The implant may be immediately loaded with a relieved provisory prosthesis.

When I placed HTR® (at time of extraction), can I put in implants later on, and how long do I have to wait then?

Yes, and this shows the superior qualities of HTR® compared to other alloplasts and bone substitutes. HTR® consists almost entirely of co-polymers and is very porous. HTR® only takes 10% of the volume to be filled, due to its hollow structure. The 90% of the space in between and around the beads become totally filled with normal bone. Drilling openings to put in an implant goes as easy as in normal bone. We do recommend however to respect the necessary healing time of 12 to 15 months between HTR® placement and loading of the implants. However to gain time and additional procedures, putting in implants at time of extraction and/or HTR® placement seems more logical.
